When filling out PDF forms, please note that information will not be saved by Adobe Reader. You can either fill out the form and then print it, or print out the form and then fill it out. In both of those cases, you can then hand-deliver or send the physical document.

Only if you have software that edits and saves PDFs (NOT Adobe Reader), you can edit the form, save it, and send the electronic version.
